Respected Sir,
I, the undersigned petitioner, most respectfully submit the following for your kind consideration and necessary permission:

That I, ABC . D/O _________M., resident of House No. ——–, Street 1B, Block-C, Sector-05, Airport Housing Society, Raja Zameer Road, Near Malik Associate, Rawalpindi, am the unmarried daughter of ________ S/O XYZ , who is presently suffering from severe mental illness and complete physical incapacitation.

That __________XYZ. is a retired Civil Carpenter from Atomic Energy, having retired in August 2005, and his only source of income is his monthly pension. He is now completely bedridden, unable to move, speak, or understand, and requires full-time medical care and supervision.

That due to his current condition, he is incapable of managing his own affairs, including financial, legal, and personal matters. Withdrawal of pension, biometric verification, and general management of his property and well-being have become impossible without proper legal guardianship.

That I have been diligently taking care of my father along with my mother, ensuring all his medical, financial, and day-to-day needs are met. I now seek legal guardianship of both his person and property in order to safeguard his rights, ensure proper medical treatment, and manage his pension and other financial matters lawfully.

That I intend to file a petition before the Hon’ble District & Sessions Judge/Guardian Judge, Rawalpindi under Sections 29 and 30 of the Mental Health Ordinance, 2001, read with relevant provisions of the Guardians and Wards Act, 1890, for the appointment as guardian of the person and property of my father, Niaz Muhammad.

That as required under Section 30 of the Mental Health Ordinance, 2001, your kind permission is sought prior to initiating such proceedings before the competent court of law.
PRAYER:
In view of the facts stated above, it is most respectfully requested that the permission may kindly be granted to the undersigned to institute a petition before the Hon’ble Court for the appointment of a guardian of the person and property of Niaz Muhammad S/O Faqeer Muhammad, under Sections 29 and 30 of the Mental Health Ordinance, 2001.
